Medical coders provide the diagnostic codes and procedure codes that apply to the patient’s visit. If the codes do not match, a claim may be denied. The insurance company will more than likely say that the treatment was not medically necessary. That is why it is so important for a medical coder to be precise.

A medical biller obtains the correct codes from the medical coder. He or she uses the codes to fill out a claim form. The claim is submitted to the insurance company, generally in the form of an electronic claim. It is important that the medical biller comply with the requirements of each insurance company. Many have specific guidelines that must be followed. The claim could be delayed or denied, if the claim form is not filled out properly and according to the insurance company’s regulations.

Medical billing software is often used in medical claims processing. The software saves time and eliminates common mistakes. Medical billing software allows medical coders to look up diagnostic codes and procedure codes via the software rather than in a manual. The software also checks databases to ensure that the diagnostic codes and procedure codes match up, eliminating the denial of claims based on discrepancies.

Medical Claims question by spring: My insurance is not paying my medical claims, what steps should I take?
Help, I live in CA. I had a laparoscopy done last year and some of my claims were paid by my insurance (most of the ones that pertained to the hospital) but the other claims r being denied. They denied my appeal and said they are not going to pay my Dr, my Anestesiologist, and a couple of other ones for lab work. I’m in the process right now of sending in some paper work for an IMR (independent medical review) I guess this is the next step after an appeal is denied because they said it was “not medically necessary” I was in alot of pain and i had to had a cyst removed. I’m seriously frustrated and upset because I’m trying to keep up w/ my payments but it’s impossibe. My medical bills will add up to about $ 20,000 (i went out of network too). Is there any advice u guys can give me- is there an easier way to get around this to make them pay? Any advice would b greatly appreciated.

is any part of your claims work related ? if not i would talk to a lawyer and get some impartial free advice from them, if it is work related you can get an injury attorney involved who deals diectly with the insurance companies , if they refuse to pay then the attorney would have to take them to court on your behalf.
